Step 2: Complete the forms and save and rename as your LAST NAME
Step 3: Send via email to: girlsequipment@usalacrosse.com
Once USA Lacrosse receives your completed forms with all signatures and coach's contact info, they will sign and return the waiver to you and your daughter's coach. Please allow 2 business days for USA Lacrosse to return the completed waiver. This waiver only applies for girls playing in the 1-8 grades with an established Rx eyewear need and only allows for specific models/standards as listed in the waiver itself.
